What Is the 75/25 Initiative?
India’s 75/25 Initiative: Tackling Hypertension and Diabetes with Urgency: Launched on World Hypertension Day 2023, the 75/25 Initiative is one of India’s most ambitious public health campaigns to date. It aims to provide standardised treatment to 75 million people living with hypertension and diabetes by December 2025. As of March 2025, the government has already reached 42.01 million hypertension patients and 25.27 million diabetes patients—an impressive sign of progress in managing non-communicable diseases (NCDs).
Understanding the Threat of NCDs
Non-Communicable Diseases, or NCDs, include hypertension, diabetes, and cancers, which are not spread from person to person but are rising rapidly due to lifestyle and environmental changes. Recognising this threat, India launched the National Programme for Prevention and Control of NCDs (NP-NCD) in 2010. This programme focuses on early screening, timely diagnosis, and affordable treatment to prevent complications like heart attacks, kidney failure, or strokes.
Screening and Tech-Based Monitoring
The NP-NCD strategy uses population-based screening for everyone aged 30 years and above. It also conducts opportunistic screening at every level of healthcare—from primary centres to tertiary hospitals. A critical aspect of this effort is the National NCD Portal, which records screening data and helps track patient progress. Teleconsultation services also connect patients in remote areas with specialists, making digital healthcare a vital part of the treatment pipeline.
Mass Screening Campaign and HWCs
In early 2025, the Health Ministry rolled out a nationwide NCD screening campaign, targeting 100% of the eligible population by March 31, 2025. This is done through India’s expanding network of 150,000+ Health and Wellness Centres (HWCs). These centres are not just for treatment—they focus on awareness, prevention, and lifestyle counselling, promoting exercise, healthy eating, and regular check-ups as part of daily life.
Building a Healthier Future
Looking ahead, the government wants 80% of diagnosed individuals to be on continuous treatment by 2025. That’s not possible without private hospitals, NGOs, and academic experts stepping in to support government efforts. The broader goal is to build a healthier society through early action and community engagement, where disease prevention becomes a shared responsibility. This marks a shift in India’s public health approach—from curative to preventive care.
